The Dublin District Coroner's Court returned a verdict of medical misadventure in an inquest into Malak Thawley's death last month.
A hospital report revealed last year that the bleeding was caused accidentally by a sharp instrument early in the operation and that the woman's death could have been prevented had the mistake been spotted earlier.
It concluded that delayed decision-making, inexperience and poor communication during the procedure were factors in the tragedy.
"My wife, my pregnant wife, my best friend died," Alan Thawley said last year. "This perfectly healthy beautiful woman just died. No warning."
