Grahame, who had been suffering from anorexia for several years, passed away on Friday 8 April.
Nikki was admitted to the hospital on March 24 and left the facility on April 8.
Announcing the news, her agent wrote:
"It is with immeasurable sadness that Nikki Grahame passed away in the early hours of Friday 9 April 2021.
"Please respect the privacy of Nikki's friends and family at this tragic and difficult time."
The star was known for her appearance on the reality show Big Brother. Fans were incredibly fond of her bubbly persona and sense of humour.
In a tweet, former Big Brother host Davina McCall described Grahame as "the funniest, most bubbly, sweetest girl".
In an emotional post on Instagram, Nikki's ex-boyfriend Pete Bennett paid tribute to the star, and raised a bottle of wine to her memory.
